By signing this form you confirm that you have reviewed your staging or live URL and approve it to 'go live' as is. After this form is signed your 30 day warranty period will commence, during which you may raise any further bugs, issues, or content changes.

If you have opted for one of our Website Care Plans, you will receive more information about maintenance and reporting separately.

Approval*
- The website is approved and meets all the requirements outlined in the original proposal, or agreed variations. Regional Business Toolkit has delivered a fully functioning and finished product.
Forms*
- I have tested any online forms, and all submissions are being received my me or my team.
Testing*
- I have been given sufficient time to test the functions of the website.
Warranty*
- I understand that Regional Business Toolkit provides a warranty period of 30 days following the launch. Regional Business Toolkit will fix any bugs or issues, as far as practical, within the warranty period. I understand that any requests for support, bug fixes, or revisions after that date may be billed as additional work.
Future Issues*
- Over time, certain aspects of the website may become unstable. Compatability with future browsers, unknown customer input, updates to software, and other activity may impact the website over time. I understand that Regional Business Toolkit is not responsible for these types of issues, but can help proactively prevent them as part of a Website Care Plan or other support contract.
Indemnity*
- Over time, certain aspects of the website may become unstable, or third-party services that the site is dependent upon (such as hosting, payment gateways, or custom functionality) may change or become unavailable. I agree that issues of this nature are not the responsibility of Regional Business Toolkit and hereby agree to hold harmless and indemnify Regional Business Toolkit, it's officers, agents and employees from any liability, loss, damages, costs, or expenses which are sustained, incurred, or required arising out of the use, operation, or malfunction of the website.
Maintenance*
- Regional Business Toolkit offers maintenance and support (eg Website Care Plans). Unless the website is covered by one of these support agreements, I understand that any maintenance and support for the website is my responsibility, and that requests for suport will scheduled as additional work.
